line-height: 14.25pt; mso-margin-top-alt: auto; mso-margin-bottom-alt: auto;\"><span style=\"color: black; mso-bidi-font-size: 12.0pt; mso-fareast-font-family: 'Times New Roman'; mso-bidi-font-family: 'Times New Roman';\"><span style=\"font-size: small;\"><span style=\"font-family: Times New Roman;\">A certified copy of birth certificate (long form), original certificate of birth (does not have to have seal), birth registration card or certificate issued by a state or county bureau of vital statistics with raised agency seal, birth card (short form).<\/span><\/span><\/span><\/div>\n<\/li>\n<li>\n<div class=\"MsoNormal\" style=\"margin: 0in 0in 0pt; line-height: 14.25pt; mso-margin-top-alt: auto; mso-margin-bottom-alt: auto;\">\u00a0<\/p>\n<ul>\n<li>\n<div class=\"MsoNormal\" style=\"margin: 0in 0in 0pt; line-height: 14.25pt; mso-margin-top-alt: auto; mso-margin-bottom-alt: auto;\"><span style=\"color: black; mso-bidi-font-size: 12.0pt; mso-fareast-font-family: 'Times New Roman'; mso-bidi-font-family: 'Times New Roman';\"><span style=\"font-size: small;\"><span style=\"font-family: Times New Roman;\">Out-of-state driver&#8217;s license with a photo which clearly identifies the individual&#8211;an official copy of driving record from last state of issuance will be required when the applicant has lost his out-of-state license. NOTE&#8211;Any foreign driver&#8217;s license must be accompanied by proper immigration documents.<\/span><\/span><\/span><\/div>\n<\/li>\n<li>\n<div class=\"MsoNormal\" style=\"margin: 0in 0in 0pt; line-height: 14.25pt; mso-margin-top-alt: auto; mso-margin-bottom-alt: auto;\"><span style=\"color: black; mso-bidi-font-size: 12.0pt; mso-fareast-font-family: 'Times New Roman'; mso-bidi-font-family: 'Times New Roman';\"><span style=\"font-size: small;\"><span style=\"font-family: Times New Roman;\">Social Security card or official written documentation verifying the Social Security Number from the Social Security Administration or specifying that no number has been assigned or that the applicant is ineligible for an SSN.<\/span><\/span><\/span><\/div>\n<\/li>\n<li>\n<div class=\"MsoNormal\" style=\"margin: 0in 0in 0pt; line-height: 14.25pt; mso-margin-top-alt: auto; mso-margin-bottom-alt: auto;\"><span style=\"color: black; mso-bidi-font-size: 12.0pt; mso-fareast-font-family: 'Times New Roman'; mso-bidi-font-family: 'Times New Roman';\"><span style=\"font-size: small;\"><span style=\"font-family: Times New Roman;\">Student I.D. card for a Louisiana college or university when accompanied by a 100% fee paid receipt for the current semester.<\/span><\/span><\/span><\/div>\n<\/li>\n<li>\n<div class=\"MsoNormal\" style=\"margin: 0in 0in 0pt;
